The Fitness and Wellness Discount Plan is not a health insurance policy. It provides discounts at certain health care providers for various fitness and wellness services. DentaQuest does not make payments directly to the providers.

Plan Benefits. To access your plan benefits, just present your ID card at the vision care center, the doctor’s office, or any of our other plan providers. Your discount will depend on the particular service you are provided or the product you purchase. You will receive your discount directly from the plan provider and pay your bill at the time of service.

JMH Health Plan Members. As an added benefit to the JMH Health Plan and at no additional charge, JMH members are automatically enrolled in this program. The JMH website is www.jmhhp.com.
