Complaints and Appeals: If you are not happy with service in the Idaho Smiles program, please call Idaho Smiles Customer Service at 800-936-0978 to let us know. You will first hear back from Idaho Smiles Customer Service about your complaint or appeal within two days, and about how it has been resolved within 30 days.
Fraud Reporting: If you feel that something has been done wrong by another participant or provider in the Idaho Smiles program, we would like for you to let us know. You can call Idaho Smiles Customer Service 800-936-0978 to tell us the information.
